Job Description:
A company is looking for a Nurse Practitioner for Virtual
Primary Care.
Key Responsibilities:
Provide virtual telehealth services, assessing patient needs and
delivering appropriate clinical guidance
Manage a panel of primary care patients, offering comprehensive
care including acute and chronic disease management
Collaborate with a supervising physician and work with a
multidisciplinary team to develop care plans
Required Qualifications:
At least 5 years of primary care experience as a Nurse
Practitioner
Licensed Nurse Practitioner in good standing with the medical
board
Current and active board certification by a national certifying
body
Experience managing a panel of patients in a primary care
setting
Must maintain active Medicare status
